Labiaplasty – Minora Reduction

Labia minoraplasty, also known as labia minora reduction, is a surgical procedure designed to reshape and reduce the size of the inner lips of the vulva, known as the labia minora.

Who is a candidate for labia minora reduction?

Labial appearance is diverse and labia minora come in all shapes and sizes. Every individual is unique, and there is no “one-size-fits-all” when it comes to the human body. However, some patients experience labial discomfort during activities such as exercise (e.g. riding a bike) or sexual intercourse. Alternatively, they may feel self-conscious about the appearance of the labia minora. Whether functional or aesthetic concerns, these patients may be a candidate for labia minora reduction. Labia minora reduction is a common procedures performed by Dr. Desrosiers.
The initial consultation for all vaginal plastic surgery is conducted with one of our female physician assistants. During the consult, she addresses the patient’s functional and aesthetic concerns. If the patient wants to learn more or proceed with the procedure, they are booked for a same day follow-on pre-operative consult with Dr. Desrosiers who will discuss the surgical plan.

How is a labiaplasty performed?

Dr. Desrosiers employs a meticulous approach to labiaplasty, prioritizing both aesthetic outcomes and the preservation of natural functions. Dr. Desrosiers utilizes a modified Alter wedge-type excision technique. This technique involves careful removal of a v-shaped piece of tissue from the lower edge of each labia minora. Following this removal, the free edge of the labia minora is sutured back into place on the vulva. This approach aims to maintain the natural contours and functions of the labia while addressing concerns related to size or asymmetry.

Note, the Alter wedge technique is commonly preferred over simple amputation of labial tissue, or the “trim” procedure. A “trim” procedure can result in an unnatural appearance. Moreover, if there’s a hypertrophic scar tissue from a “trim” procedure, dyspareunia can result.

Is a labiaplasty painful?

Using our approach, generally no. Why? Because we use a long-acting local anesthetic – e.g., Exparel® or Endura-KT® – that blocks pain for about 48-72 hours. Using this approach, most patients experience little to no pain. The utilization of long-acting local anesthetics has truly revolutionized the patient experience for labiaplasty. By incorporating these advanced local anesthetics into the procedure, we’ve significantly enhanced patient comfort. We also offer patients the option of a follow-on long-acting anesthetic 2-3 days after the first long-acting anesthetic. Although few people request a second injection, we want patients to be aware of this option to alleviate concerns about post-procedure pain. Patients are also given prescription pain medicine to have on hand, if needed. In our experience, most patients don’t end up taking it.

What is recovery like?

During the procedure, dissolvable stitches are carefully placed, eliminating the need for later stitch removal. Following the surgery, patients are often advised to wear compression garments, such as snug leggings, along with several pads. This combination serves to maintain pressure on the surgical site, reducing the risk of hematoma formation. After approximately 72 hours, patients can typically discontinue using pads, while compression garments are usually recommended for an additional week to support proper healing and minimize swelling. This post-operative care regimen aims to optimize recovery and promote favorable outcomes following the procedure.

Can a labiaplasty be done in the office?

Most of the time, a labiaplasty is done in the office under local anesthesia (usually with long-acting anesthetic agent for ideal patient comfort). However, when combined with other procedures or because of patient preference, conscious sedation or general anesthesia may also be options.

What are the restrictions after a labiaplasty?

Generally, with the long-acting local anesthetic that we use, most patients go back to work the next day. However, patients need to refrain from vaginal intercourse and tampon use for 4-6 weeks. Most other activities can be resumed in 1-2 weeks.
