How to Get Rid of Under Eye Bags: Causes, Treatments, and the Best Long-Term Solutions

Under-eye bags, also commonly referred to as bags under the eyes, are a common aesthetic concern among both men and women. These bags can make you appear tired, older, or stressed, even when you are well rested. While concealer and eye creams may temporarily improve the appearance of puffiness, they often do not address the underlying cause.

Fortunately, advances in aesthetic medicine and cosmetic plastic surgery now offer effective options for treating under-eye bags and restoring a more refreshed, youthful appearance.

What Causes Under-Eye Bags?

Before choosing the right treatment, it is important to understand why under-eye bags develop.

In many cases, they are a natural part of the aging process. As we age, the tissues and muscles supporting the lower eyelids gradually weaken. This can allow the normal separation of the orbicularis muscle from the cheekbone and allow the lower-eye fat that normally cushions the eye to protrude forward, creating a puffy appearance. At the same time, the skin loses collagen and elasticity, making under-eye bags even more noticeable.1

Genetics also play a significant role. Some people develop under-eye bags at a young age simply because the condition runs in their family.

Other contributing factors include lack of sleep, allergies, excess salt intake, dehydration, smoking, and fluid retention. While these lifestyle factors may worsen puffiness, they are not always the primary cause.

Can Home Remedies Help Under-Eye Bags?

If your under-eye bags are mild or related to temporary swelling, several at-home remedies may help reduce their appearance.

Applying a cold compress for a few minutes can decrease swelling by constricting blood vessels. Staying hydrated, getting adequate sleep, reducing sodium intake, and treating seasonal allergies may also help minimize puffiness.

Many skincare products marketed for under-eye bags contain ingredients such as caffeine, retinol, peptides, or vitamin C. These ingredients may temporarily reduce swelling or improve skin quality, but they generally cannot correct significant fat bulging or loose skin.

For patients with true age-related under-eye bags, professional treatment typically provides the most noticeable and lasting improvement (Healthline, 2026).

Professional Treatments for Under-Eye Bags

The best treatment for under-eye bags depends on what is causing the concern.

During a consultation, we can determine whether the primary issue is excess fat, volume loss, skin laxity, skin quality, or a combination of these factors. Identifying the underlying cause is important because different concerns require different treatment approaches.

Lower Blepharoplasty

Lower blepharoplasty, commonly known as lower eyelid surgery, is considered the gold standard for treating prominent under-eye bags caused by fat protrusion.

During the procedure, excess fat may be removed or repositioned, and loose skin can be tightened to create a smoother transition between the lower eyelid and the cheek.

Unlike temporary treatments, lower blepharoplasty provides long-lasting improvement and can dramatically refresh the appearance without changing your natural facial expression. The goal is not to make patients look different. It is to help them look more rested and rejuvenated.

Injectable Fillers

Not every patient with under-eye concerns needs surgery.

In some cases, the issue is not a true under-eye bag but rather a hollow area beneath the eyes known as the tear trough. Carefully placed hyaluronic acid filler can restore lost volume, soften shadows, and create a smoother contour between the lower eyelid and cheek.

Because the under-eye area is delicate and anatomically complex, hyaluronic acid filler injections should only be performed by an experienced injector with extensive knowledge of facial anatomy to help ensure safe, natural-looking results.

Can Lipoderma Help With Under-Eye Bags?

An alternative to hyaluronic acid fillers is Lipoderma.

Lipoderma can be an option for addressing localized fat and improving the overall contour of the under-eye area. By targeting the junction between the orbicularis muscle and fat pads and the orbital rim and zygomatic muscle insertions, Lipoderma can help create a smoother, more refreshed appearance without dramatically changing your natural features.

Depending on your anatomy and aesthetic goals, Lipoderma may be incorporated into a personalized treatment plan to help achieve a more defined and rejuvenated appearance.

Platelet-Rich Plasma (PRP)

For patients who want to improve the quality of the skin around their eyes without surgery, Platelet-Rich Plasma, or PRP, is a regenerative treatment option.

PRP uses the patient’s own blood to isolate growth factors that stimulate collagen production and promote tissue repair. When injected or applied around the under-eye area, PRP can improve skin thickness, reduce the appearance of fine lines, and brighten dark circles caused by thin skin.

While PRP will not remove large fat pads, it can noticeably improve overall skin quality and complement other treatments.2

Microneedling

Microneedling is another minimally invasive treatment offered in our practice that helps rejuvenate the delicate skin beneath the eyes.

By creating controlled micro-injuries in the skin, microneedling stimulates the body’s natural healing process and encourages new collagen and elastin production.

Over time, patients often notice smoother skin, improved firmness, and a reduction in fine lines and crepey texture around the eyes. Microneedling can also enhance the absorption of professional skincare products and is frequently combined with PRP for additional collagen stimulation.

Chemical Peels

Chemical peels can also play an important role in refreshing the under-eye area and improving the overall appearance of the skin.

Specially selected superficial peels help exfoliate damaged skin cells, improve pigmentation, smooth fine lines, and stimulate healthy cell turnover.

Although chemical peels will not eliminate true under-eye bags caused by fat herniation, they can significantly improve skin tone and texture, helping the eyes appear brighter and more youthful. Chemical peels are often incorporated into a comprehensive treatment plan alongside other aesthetic procedures.

Why a Customized Treatment Plan Matters

One of the biggest misconceptions about under-eye bags is that there is a single treatment that works for everyone. In reality, every patient’s anatomy is different.

For example, someone with significant fat herniation may benefit most from lower blepharoplasty, while another patient with volume loss may achieve beautiful results with filler alone.

Patients with thin, crepey skin may be better candidates for PRP, microneedling, laser resurfacing, or chemical peels. Many patients achieve the best results by combining several treatments to address multiple aspects of aging simultaneously.

That is why an in-person consultation is so valuable. A thorough facial evaluation allows your provider to identify the true cause of your under-eye concerns and recommend the treatment that may produce the safest and most natural-looking results for your individual anatomy.
